Problème avec un formail

Bonjour, j'ai trouvé un formulaire de contact "formail.zip" que je mets à votre disposition ici : http://www.benefweb.com/formail.zip. Je l'ai installé, aucun problème, sauf que lorsque je l'essaye, je reçoit quelques secondes après le message suivant dans moi boîte mail :
"Message depuis votre site web:". Et c'est vide... Je ne vois ni le nom de l'expediteur, ni son adresse mail, ni son commentaire... Quelqu'un saurait me dire d'où cela provient? Merci.

Bonjour,

Je pense que le peu d'informations que vous nous donnez n'aidera pas à répondre.

Pouvez vous poster la source de votre code php, en remplacant votre adresse E-Mail par une fausse, afin que nous puissions y jeter un oeil ?

A quelle adresse peut on voir le formulaire en question ?
